Do I qualify for CalFresh benefits?

Completing the CalFresh application
If you are eligible to apply for CalFresh benefits, the next step in process is completing and submitting the application. Click here to download the application (PDF)
IMPORTANT! You will need copies of the following documentation to complete your application:
- Identification for all members of the household (driver's license, state ID card, birth certificate, library card, etc.)
- Proof of income (pay stubs from last two months, social security, unemployment award letter, etc.)
- Proof of residency (any mail with your name and current address)
- Proof of rent (rent or mortgage reciept, copy of lease, etc.)
- Proof of utilities (gas, electric, telephone, water, and trash bills)
- Child care costs (for expenses incurred in order to go to work)
- Medical expenses (if you have a member of the household over 60 or disabled)
Once you have completed the application, fax the application and all required documents to 858-768-7438 (ATTN: CalFresh Outreach). You can also mail or deliver your completed application to
